First Appointment Information

  • Please fill out all intake paperwork prior to the day of your appointment. For example, if your appointment is on a Thursday, please have the paperwork filled out by Wednesday. This allows time for your provider to review the paperwork prior to meeting with you. Our front office will email you the intake form after scheduling.

  • Intake appointments can take anywhere from 60-90 minutes depending on your concerns and the complexity of the treatment plan. You will be asked questions about your mental health history and current symptoms, family background, work and educational background, and medical history. 


  • Please make payment arrangements prior to your first appointment. Provide our office with insurance documentation and/or a credit card to have on file. 

  • At the end of your appointment, your provider will discuss diagnoses and a plan to address your concerns. This could include prescribing medications, recommending behavioral changes, making therapy recommendations, or other interventions to support your general wellbeing. This process is collaborative and a time to ask questions, share concerns, and get clarification on the information provided.
